Coming home is only the beginning
Leaving hospital is often a welcome step, but the period afterwards can bring new challenges.
Someone may be less mobile or less confident than before. Medication may have changed. Appointments, physiotherapy or occupational therapy may be involved. Family members may suddenly find themselves responsible for coordinating several aspects of someone's recovery.
Fond can help put the right support in place for the individual as they return home.
Supporting recovery, not creating dependence
Care might initially be more substantial and then reduce as someone recovers their strength and confidence.
Our role is to understand what support is needed now, work alongside the other people involved in someone's recovery and adapt care as circumstances improve or change.
The aim isn't simply to get someone home. It's to help them live well once they're there.
